How to Be Admitted

Calvert Hospice admits patients who have an illness that is progressing and/or a life expectancy that is limiting. If you think you or a family member may be Hospice appropriate, schedule a meeting with one of our representatives. We can provide you with the necessary information to make a decision about Hospice care.

Admission Criteria
 
• Patient is diagnosed with a life-limiting illness and is deemed Hospice appropriate by their 
  attending physician
• Patient is interested in seeking care rather than cure
• Patient and family desire Calvert Hospice services

Referral Information

Patients are generally referred to Calvert Hospice by their primary care physician. However, the process can be initiated by other health care professionals, family members, friends, clergy or by the patients themselves. To reach the Calvert Hospice Admissions Department, 24 hours a day, call 410-535-0892.
