Criteria by Diagnosis (Cancer)

Patients do not have to have all criteria to be eligible for care

• Oxygen use
• Impaired nutritional status
• Cancer progression

o   Change in mental status

o   Decline in activity level

o   Anorexia

o   Ascites and/or edema

o   Bleeding

o   Coma

o   Dyspnea on exertion

o   Jaundice

o   Metastases

o   Palpable increasing mass

• Failure of chemo/radiation
• Patient refusing treatment
