Failure to Thrive

  • Advanced age
  • End stage disease/treatment discontinued
  • Documented cancer with metastases
  • Life limiting condition
  • Impaired nutritional status
    • Aspiration
    • Dehydration/hypovolemia
    • Dysphagia
    • Impaired cough reflex
    • Nausea/vomiting uncontrollable
    • No response to nutritional support despite adequate caloric intake
    • Oral intake insufficient
    • Progressive or 10% weight loss in the past 6 months
    • Pt declining enteral/parenteral support
    • Unable to feed self
    • Unable to swallow
    • Unwilling to take foods/fluids
  • Inpatient hospitalizations (frequent)
  • Laboratoy studies incicating decline
    • Serum albumin <2.5 gm/dl
  • Major organ failure
  • Multiple end stage diseases
  • Multiple ER visits
  • Pt/family have elected palliative care
  • Requires medication for pain/symptoms
    • Frequent medication changes
  • Sudden onset of catastrophic illness
