Failure to Thrive
- Advanced age
- End stage disease/treatment discontinued
- Documented cancer with metastases
- Life limiting condition
- Impaired nutritional status
- Aspiration
- Dehydration/hypovolemia
- Dysphagia
- Impaired cough reflex
- Nausea/vomiting uncontrollable
- No response to nutritional support despite adequate caloric intake
- Oral intake insufficient
- Progressive or 10% weight loss in the past 6 months
- Pt declining enteral/parenteral support
- Unable to feed self
- Unable to swallow
- Unwilling to take foods/fluids
- Inpatient hospitalizations (frequent)
- Laboratoy studies incicating decline
- Major organ failure
- Multiple end stage diseases
- Multiple ER visits
- Pt/family have elected palliative care
- Requires medication for pain/symptoms
- Frequent medication changes
- Sudden onset of catastrophic illness
