Transitions vs Hospice Care

Hospice

Transitions is a non-medical program designed for clients with an expected prognosis of 12 months or less.

Hospice, as defined by Medicare, is designed for patients with an expected prognosis of 6 months or less.

Transitions may be appropriate for clients who are diagnosed with a life limiting

diagnosis but are currently pursuing curative treatment.

Hospice may be appropriate for patients who have a life limiting diagnosis and are no longer pursuing treatment.

 

The Transitions coordinator connects clients to resources in the community.

Hospice care can be provided to patients with a variety of terminal diagnoses, not limited to cancer.

Transitions volunteers are provided for companionship and support. Volunteers can also assist with running errands or transportation to appointments.

Hospice may be helpful for patients who are having an increase in symptoms such as dyspnea, pain, or who require more assistance with ADL’s

Transitions provides a link to  Hospice care if a client does become appropriate and chooses Hospice care.

 

Hospice provides patients with an individualized team including a nurse, home health aide, social worker, chaplain, and volunteer, in order to address the multidisciplinary issues that arise during end of life care.
